The review comments should be part of the same git branch, as special commits that only have a commit message. Then you can add new commits in response to the review, and it’s easy to see what you changed and verify that the starting point is what was reviewed.

I'm really surprised we aren't seeing a more decentralized git usage being adopted.

Their are projects which have build a system which allows issues to be tracked in a special git repository:
