So why aren't these optimizations being done "for real", with the intent of being packaged for normal distros? It's great to know that in theory it can be done, but it's of little benefit if no one actually does it...

Actually, this stuff does look like it's on the way to becoming "real." Ted Ts'o and Keith Packard (filesystem and X) were in the crowd, and both sounded interested in accepting this work, or something based on it, into the "real" versions of the software. The actual readahead program is supposed to be on the Moblin site soon -- so Fedora, Ubuntu and the rest can download and integrate it if they want to.

I have been booting in a rough eight seconds (GRUB->X) for months without the problematic issues of readahead and deferring startup to the 'background'. My system is loaded and synchronized in 8 seconds flat, without tearing out subsystems and destroying so many internals and hacks on Fedora.
