The Ägypten project added S/MIME (and PGP/MIME support) to KMail
during 2001-2002. It took a while until it made it into the official KDE releases and the setup was not easy then. Still the modular design (involving GPG) with the gpg-agent is a bit more secure than direct calls of gpg.
The design made it possible to use KMail with crypto integrated circuit cards (aka smartcards) to have fully qualified signatures according to the German

No "plug-ins" anymore and a lot more GUI functionality to deal with S/MIME certificates and the setup.
