The Ägypten project added S/MIME (and PGP/MIME support) to KMail
during 2001-2002. It took a while until it made it into the official KDE releases and the setup was not easy then. Still the modular design (involving GPG) with the gpg-agent is a bit more secure than direct calls of gpg.
