No, it has been on the agenda for many years, starting with Enrico Zini's Debian Community Guidelines document from 2005. There was a BoF at DebConf13 about forming a CoC and it was discussed on the lists a fair bit.

It's been on the agenda for a long time but I vaguely recall some specific behavior was the trigger for it's creation though for the life of me I can't remember what it was or even if it was specific to the Debian project.

I seem to recall some incident maybe a decade ago, where there was a major conflict at a DebConf, and someone ended up being ejected or something. I wasn't an attendee, but the discussions and arguments over the event took over the mailing lists for a while. I've mercifully forgotten the details, and don't think they're really important, but I suspect it was one of the big initial triggers that eventually led to this.
