In "projects have found success with open source tools for translating application strings and in-program help messages", compare MediaWiki's Translate, used to translate MediaWiki in almost 300 languages on translatewiki.net: "none of those features are available in the systems that projects use to maintain their documentation", but MediaWiki (and Wikimedia on Meta-Wiki, as well as KDE) uses Translate also for its documentation, because it has none of the problems you mention besides many features other systems lack, and further improvement to the interface is in the works.
