I use a lot of GTD, and currently use a text file (well, two - one for work and one for personal). Switching to a tool would only be worthwhile, IMO, if it gave me something better - like automatic syncing with my mobile phone, via a web repository and an Android app.

I use Shuffle on my Android phone.

It does projects, contexts, and since the last release it also syncs with Tracks (I use it with http://www.gtdify.com/).
