For windows see the one Bruce Schneider started and oversees. For GNOME there's Revelation (and the older gringotts, which is pretty unmaintained and isn't as nice to use, IMO).

Personally I get firefox to remember all my web passwd's, as well as having them in revelation, they are all unique and 90+% of them it wouldn't matter if they were compromised for a few days. Like OMG someone stole my mailman admin passwd, stop the press.
