Block layer performance problems

There are some performance issues with the block layer.

Requests are broken down to individual blocks
Most of them start out larger
Elevator must merge them back together
Processing overhead, and lots of buffer heads

Raw I/O is synchronous
Due to 64KB chunking of requests

Single-queue drivers limit parallelism
Multiqueue capability exists now
