There is a lot to lose when you centralize something that should really be left to the application authors. The app icon is the app's identity. Sure it's more difficult to convince the upstream to take your work despite it not being created by an algorithm, but taking away a project's identity in the name of policing the aesthetics of the overview is not the right approach.

I'm old enough to remember Windows 3.0. Or is it Windows 3.1? And the abundance of choices in [something-MSFT-provided].ico. Maybe within system.exe? Ok: I'm old enough to vaguely remember something.

They weren't great, I grant, and no commercial product would be willing to risk being one of the ~300 or so reasonably non-shitty provided icons.

Does Gnome and KDE not provide less than crap for their defaults? Or did they only provide crap when choosing a default icon ID was first available?

You are probably thinking about moricons.dll. In later Windows versions Explorer.exe also contained quite a collection.
