As projects get more mature it's hard to point to one feature and say "this is my major version bump." In fact, if you try to do a major version release and drive it with lofty goals of major refactoring, you're more likely to end up with a release which is years late. See Perl 6 and Python 3 for examples of this.

Or you get a project that isn't really finished, either in terms of residual bugs, omitted features, or both. See KDE 4 and GNOME 3 for examples.
