I think a counterpoint to this should be written. I mean I think every organization has had these things happen to them... and some recover and some do not. Why is it? What things work on rebuilding a community after trust has been broken.

Seconded. I think it is actually quite simple: the means, requirements and goals of the bigger organization and those of the smaller organizations of which it is made up are different, and not necessarily aligned.
