Depends on your definition of "real gain". Almost all the time I underuse the speed and memory of my 64-bit desktop machine; if I'd need more speed, i'd overclock or buy a faster desktop. For me the real gain is that now I can build and test my SW for 64-bit at home.

I imagine some people may find a real gain the ability to run unmodified kernels on top of kvm with recent cpus and the like.
