The KDE music player project Amarok (http://amarok.kde.org/) has recently released a new version 1.4.1 with built-in support for Last.fm streams. It utilizes a proxy extension written in Ruby. At this time, it appears to support radio streams for tags and groups, with a GUI for setting up and listening to custom radio stations planned for 1.4.2.

yep, i build amarok from SVN just for this - it's really cool to be able
to choose "play last.fm stream -> custom radio", enter a few band names,
hit enter - and let it play a personal radio stream. Neighbor radio is
very cool, too.
